Terms and Conditions for 30 Parameter Health Checkup

1. This Voucher entitles you to assured rewards as per the below:
• 1 Free 30 Parameter HCU and
• 1 Free Online Doctor Consultation and
• 1 Month Cult Fit Subscription along with Discount Offers worth upto Rs. 25,000.
2. This offer entitles the holder for a 30-parameter Health Checkup at participating venues in select cities of India.
3. Voucher once booked cannot be rescheduled or cancelled. If the customer fails to show up at the health centre, the voucher will be deemed to be redeemed.
4. The cost of travelling to the health center, SMS and call charges while doing the booking or any other cost associated with availing the offer shall be borne by the participants.
5. A minimum of 48 hours’ prior notice is required to book an appointment. Walk-in customers will not be entertained at the health center.
6. The list of participating health centers may be subject to change.
7. No request for change of date, time, and health center shall be entertained by the Promoter/Agency. If the successful Participant fails to show up on the date, time and health center allocated to him/her, the offer will be forfeited in its entirety at the discretion of the Promoter/Agency.
8. In case the successful Participant does not avail the Promo offer on the date and time informed by the Promoter/Agency, the Promoter/Agency shall be relieved of its obligation under the Promo. As such, the successful Participant will have no claim against the Promoter/Agency. In case the Laboratory/ Diagnostic Centre conducting the Health Check-up is not available in the successful Participant’s city of residence, then the successful Participant shall have to travel to the nearest city where the Laboratory/ Diagnostic Centre is available at his/her own expenses. All expenses including but not limited to food, travel, stay and lodging or any other miscellaneous expenses will be borne by the successful Participant only. The Organizer/Agency will not be liable to pay/reimburse any such expenses borne by the successful Participant.
9. The Promoter/Agency makes no representation or warranty of any kind, either express or implied, with respect to the Promo Offer or the laboratory / Diagnostic Centre which oversees conducting the Health Check-up under the Promo offer.
10. The promoter/Agency does not guarantee or make any representation with respect to the correctness, completeness or accuracy of the Health Check-up conducted and reports generated by the Laboratory/ Diagnostic Centre and shall not be liable to any issues/claim/query arising out of such tests conducted. Laboratory/Diagnostic Centre shall be responsible for the authenticity and accuracy of the same and any claim/query pertaining to it, shall be dealt with by the Laboratory/Diagnostic Centre only.
11. The Promoter/Agency shall not be responsible for any direct or indirect loss, injury or any other liability arising out of the Promo offer.

Terms and Conditions:  
30 - 1 Month Cult Fit Subscription Voucher
• The promotional voucher is valid for 3 Months from the date of issue.
• Voucher unlocks access on only “cure.fit Live / live.fit” memberships and is not applicable on other products on cure.fit app/website
• The products are available under the “LIVE” tab only.
• Once the voucher is unlocked it will appear under “Active Packs / Subscriptions” in your profile section. Each voucher can be redeemed only once per user.
• The voucher is applicable to be used in India only.
• This pass does not allow customer to pause the membership
• Only one voucher can be used at a time and cannot be combined with other offers.
• The voucher is not transferable and should not be resold or redeemed for cash
• The voucher cannot be returned under any circumstances.
• Usage of the voucher shall be subject to acceptance of terms and conditions by the member available on cure.fit app/web.
• cure.fit reserves the right to modify or revoke the voucher / offer at any point in time

• Under the offer the voucher holder can avail 1 Free Online Doctor Consultation (general physician) in select cities of India.
• The Free Doctor Consultation Code can be redeemed either on the partner App or Web platform only.
• Codes not valid on medicine orders and lab tests.
• Code is valid only for Doctor Consultation online through chat or via call depending on speciality and Doctor selected
• A maximum of 1 code can be used per account.
• This offer is subject to promotional availability.
• This voucher has no monetary value, is non-transferable and cannot be used in conjunction with any other promotional offers.

General Terms & Conditions
1. Only the purchases made between 1st July 2024 to 30th September 2024 (90 Days) will be considered for rewards.
2. The Participants are eligible for 1 Free 30 Parameter Health Checkup + 1 Free Doctor's Consultation + 1 Month Cult Fit Subscription + Additional Offers worth Rs. 25000.
3. This offer is for consumers only and is not a trade scheme or employee scheme for Company’s employees, retailers, dealers and/or distributors. The usage of this Voucher is incumbent upon the terms & conditions to be followed by the voucher holder/purchaser. Such conditions need to be followed by the voucher holder/purchaser.
4. The "How to Use/Redeem" instructions form a part of these Terms & Conditions.
5. The Promotion & contest shall run on the select SKUs only.
6. This offer is subject to promotional availability. This offer is not exchangeable for “cash” or for “sale”.
7. Any costs associated with entering into the promotion, including accessing the promotional website, are the responsibility of each entrant. Also, regular call and message rates will apply as per the consumer telecom operator in their respective circles.
8. Only one voucher code per booking will be entertained. Multiple vouchers cannot be booked together.
9. Only printed invoice/bill that includes in a legible manner the Store Name (Only select Outlets), Store Address, Store GST No, Date of Purchase, Invoice/Bill No, Item Names (only the participating products), and prices will be considered as an eligible receipt for the purpose of claiming rewards.
10. No handwritten bills/invoices or photocopied bills/invoices, scanned, illegible, tampered or defaced bills/invoices will be considered.
11. The invoice/bill copy should clearly mention the eligible products ie. < SKU name>
12. The invoice/bill copy should be in ENGLISH language only.
13. One single bill/invoice will be valid for one reward, regardless of the number of eligible SKUs purchased in that invoice.
14. Photocopied, torn or mutilated Vouchers will not be accepted.
15. Customer purchase invoices should have customer name and mobile number, no registration/booking will be entertained if the invoice is in the name of any other person or company/organization.
16. The customer will need to register the voucher within 15 days of their purchase or before the last date to register i.e. 15th October 2024, whichever is earlier, failing which the voucher will be invalidated and cannot be used further.
17. A maximum of one voucher can be used per person, mobile number, email id, and account, per invoice during the promotion period. Similarly, a user can redeem a maximum of one health reward per week during the promotion period.

19. Premier Sales Promotions Pvt. Ltd. and/or Company hold no legal and financial liability on unclaimed offers, incentives, vouchers, etc. beyond the applicable period of the promotional offer
20. Under this promotion, the Vouchers are being offered directly by Premier Sales Promotions Pvt. Ltd. The Company is not responsible either directly or indirectly concerning any matter pertaining to this Voucher and the prize offered with this Voucher.
21. Each Participant represents and warrants that he/she is legally competent to enter into binding contracts under applicable laws. By taking part and/or entering into the contest/ promotion the Participant warrants that all information provided by the Participant regarding the Participant's name, age, state, city, address, phone number, etc., is true, correct, accurate and complete.
22. The decision of Havells or its Agency, as regards the selection of winners and distribution of the prizes shall be final, subject only to Havells approval. No correspondence or any other claims whatsoever, in this regard will be entertained.
23. No queries, claims, dispute, complaints, or grievances shall be entertained by Havells. and its agency after 15 days from the date of the closure of the contest/ promotion period.
24. Premier Sales Promotions Pvt. Ltd. shall not be liable in any manner for any cost or expense incurred for the purpose of redemption of the Voucher.
25. If this promotion is interfered with in any way or is not capable of being conducted as reasonably anticipated due to any reason beyond the reasonable control of the Company, including but not limited to technical difficulties, unauthorized intervention or fraud, the Company reserves the right, in its sole discretion, to the fullest extent permitted by law (a) to invalidate any customer; or (b) subject to any written directions from a regulatory authority, to modify, suspend, terminate or cancel the promotion, as appropriate.
26. Premier Sales Promotion Pvt. Ltd. is not responsible for any problems or technical malfunction of any telephone or network or lines, servers or providers, computer equipment, software, technical problems or traffic congestion on a mobile network, or any combination thereof, or any other technical failures including any damage to entrant's or any other person's mobile handset or computer related to, or resulting from, participation in this promotion or the downloading of any materials related to this promotion.
27. If for any reason this promotion is not capable of running as planned because of infection by computer virus, bugs, tampering, unauthorized intervention, technical failures or any other causes beyond the reasonable control of the Promoter which corrupt or affect the administration, security, fairness, integrity or proper conduct of this promotion, Company reserves the right in its sole discretion to cancel, terminate, modify or suspend the promotion, subject to any written directions made under applicable State or Territory legislation.
28. Caution: Any attempt to deliberately damage the Company’s promotional website or the information on the website, to cause malicious damage or interference with the normal functioning of the website, or to otherwise undermine the legitimate operation of this promotion may be a violation of criminal and civil laws and should such an attempt be made, whether successful or not, the Company reserves the right to seek damages to the fullest extent permitted by law. If the Company suffers loss or incurs any costs in connection with any breach of these Terms or any other legal obligation by an entrant, the entrant agrees to indemnify the promoter for those losses, damages, and costs.
29. This voucher has no monetary value, is non-transferable and cannot be used in conjunction with any other promotional offer.
30. Premier Sales Promotions Pvt. Ltd. and/or Company will have no legal and financial liability on unclaimed offers, incentives, vouchers, etc. beyond the applicable period of the promotional offer.
31. Premier Sales Promotions Pvt. Ltd., its clients, agents, and distributors accept no responsibility, financial or otherwise for the misuse of Premier Sales Promotions Pvt. Ltd. vouchers by the participants and are not liable for any personal loss or injury and cannot be held responsible for any disagreement concerning quality/ availability of featured offers.
32. Promoter does not endorse the quality of any of the products or services being offered by the Premier Sales Promotions Pvt. Ltd or by the cashback partners. Any complaints or queries pertaining to the same would have to be taken up directly with the Premier Sales Promotions Pvt. Ltd marketing of such a product or service.
